Essential Documents and Information:

  • Referral Slip: If you’ve been referred by another dentist, please bring the referral slip along with any relevant x-rays.
  • Patient Registration Paperwork: Completing your registration forms in advance can significantly expedite your visit.
  • List of Current Medications: A detailed list of medications you’re currently taking is crucial for us to provide safe and personalized treatment.
  • Insurance Forms: If you have medical or dental insurance, bringing the necessary completed forms will help us assist you more effectively with claim processing.

Important Notices:

  • Underage Patients: All patients under the age of 18 must be accompanied by a parent or guardian during the consultation visit.
  • Medical Conditions and Medications: Please inform our office in advance if you have any medical conditions (e.g., diabetes, high blood pressure, artificial heart valves or joints, rheumatic fever) or if you are taking any specific medication (e.g., heart medications, aspirin, anticoagulant therapy) that could affect your treatment. Additionally, let us know if you require any pre-medication before dental procedures, such as antibiotics for pre-medication purposes.

X-Rays:

  • Transferring X-Rays: If you have had x-rays taken by a previous dentist within the last 6 months, you can request to have them forwarded to our office.
  • Pre and Post Operative X-Rays: Please note that it is our policy to take pre-operative and post-operative x-rays to ensure the highest standard of care. These x-rays are essential for assessing your dental health and for planning and evaluating your treatment.
